Requirements
- Death Certificate (Local Civil Registry)
- Burial Permit (City Health Office)
- Transfer Permit (if outside Lipa) (City Health Office)
- Senior Citizen or PWD I.D. (Deceased)
- Certificate of Ownership (COO)
Notes:
- Fully paid interment fees are strictly enforced.
- Less than 3 days notice clients will have to pay additional fee of Php 3,000.00
