Requirements
- Fill out the exhumation form from LTP Office (Downloadable form coming soon)
- 2 valid IDs of the owner
- Copy of Certificate of Ownership (COO)
Notes:
- Fully paid interment fees are strictly enforced.
- Less than 3 days’ notice clients will have to pay an additional fee of Php 3,000.00
